At Sunflower Care Victoria, most of the services we provide can be funded by the National Disability Insurance Scheme (NDIS).
If you have an approved NDIS plan, we are ready to work with you 1:1 to tailor your plan to get the best outcome in achieving your goals.

Support Coordination
Working through how to make the best use of your NDIS funding can be a challenge. Finding the right service providers to meet your needs, coordinating different services and keeping track of your expenses can be overwhelming.
We recognise these challenges at Sunflower Care Victoria. As your support coordinators, we can:
help you to understand your NDIS plan

support you to achieve your goals
monitor your budget and services
help you make connections to your local communities.
Daily Living Assistance
Sunflower Care Victoria can assist with a broad range of daily living activities based on unique personal needs:
• Personal hygiene and grooming
• Grocery shopping and meal preparation
• Appointment accompaniment
• Transport support
Social and Community Participation
We support participants however they engage and connect with the community. We let participants tell us what they enjoy doing, and we make it happen! Examples of activities we support include:
joining a local sports team
going to the movies with friends
arts or music class
volunteering
travelling
cooking/baking
Group Activities
Sunflower Care Victoria offers many activities which participants are welcome to join. Based on participant interest and feedback, these events promote community engagement.

Skills and Development
Skills Development Training
Sunflower Care Victoria wholeheartedly supports the development of individual life, social and vocational skills. We personalise the training to each participant with these core areas:
Life Skills: training participants to manage everyday tasks such as cooking, shopping, using public transportation
Vocational Skills: assisting participants to explore job and career prospects, and connect them with specialised training providers
Social Skills: training participants to improve interpersonal communication and social participation.
Social Work Support
Sunflower Care Victoria can provide counselling, psychosocial assessments and recommendations to help participants address complex emotional, social and practical challenges to achieve their personal goals.
Participants can meet with an accredited Social Worker by the Australian Association of Social Workers.
Community Nursing Care
Some NDIS participants are eligible for community nursing care.
At Sunflower Care Victoria, we provide in-home nursing support such as:
Medication management
Diabetes education and management
Self-managed injection education and management
Continence assessment
Wound management
Our aim is to help you manage your day-to-day healthcare needs in the comfort of your own home without the hassle of attending scheduled appointments on a regular basis. For NDIS participants and other in home nursing support can be requested or arranged upon further enquires..
Aged Care
Through the Support at Home* programs we offer a range of Aged Care services for elderly clients for all stages of health including:
Clinical Supports (continence nursing, wound care, medication management, diabetes management)
Support for Independence (social support, transport.
Our ability to communicate with our elderly clients in Mandarin, Cantonese or Hakka is valued by many.

Psychosocial Recovery Coaching
Psychosocial Recovery Coaches support participants to take control of their lives and to better manage the complex challenges of day-to-day living.
We work collaboratively with participants, their families, carers and other service providers to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports.
Our aim as Psychosocial Recovery Coaches is to help build an individual’s capacity across all areas of the person's life.
